Vitamin D3
Vitamin D3 has strong evidence for protecting bone health during menopause, with multiple studies showing improved bone density when combined with calcium. Most menopausal women test deficient, making this one of the most important supplements to consider during this transition. Blood testing reveals your individual needs, but most women benefit from 2000-4000 IU daily when your bones need every advantage.

Overall: Strong evidence
Randomised controlled trials
Multiple randomized trials show vitamin D3 plus calcium significantly improves bone mineral density in postmenopausal women compared to placebo.
Large population studies consistently link higher vitamin D levels with stronger bones and fewer fractures in older women.
Systematic reviews of 20+ studies confirm vitamin D supplementation reduces fracture risk by 15-20% when combined with adequate calcium.
Menopause-specific trials
Several studies specifically in menopausal women show D3 supplementation slows bone loss during the critical early postmenopausal years.
What we do not know
Most research combines vitamin D with calcium, so we don't know how effective D3 alone is for bone health. Optimal doses for menopausal women specifically haven't been established in dedicated trials. Studies rarely include women over 70, leaving postmenopausal dosing unclear. The relationship between vitamin D levels and hot flashes remains unproven despite promising observational data. We don't know if higher doses beyond 4000 IU provide additional benefits for menopausal symptoms.
How it is used
Common dose range
1000-5000 IU daily
Notes on dosing
Always taken with K2 (MK-7 form). Take with fatty food for absorption.
Get it from food first
Food sources are better absorbed than most supplements and come with co-factors that support the same pathways. If you eat two or three of these consistently, you may not need a supplement at all.
Fatty fish
400-1000 IU per 3.5 oz serving
salmon, mackerel, sardines are richest sources
Fortified milk
100-144 IU per cup
check labels as fortification varies
Egg yolks
40 IU per yolk
pasture-raised eggs contain more
Mushrooms
400 IU per cup
UV-exposed varieties like maitake
Mediterranean diet
includes fatty fish and fortified foods that naturally boost vitamin D intake
Nordic diet
emphasizes fatty fish consumption which provides natural vitamin D along with omega-3s
What depletes Vitamin D3
Limited sun exposure, especially in northern climates or winter months. Aging skin produces 75% less vitamin D than younger skin. Excess caffeine and alcohol can interfere with absorption. High fiber intake may reduce absorption if taken simultaneously with vitamin D supplements.
Interactions and cautions
moderate
Thiazide diuretics — High-dose vitamin D3 combined with thiazide diuretics (commonly prescribed for blood pressure) may increase calcium levels too high.
Inform your doctor before starting vitamin D3 if you take thiazide diuretics. Routine doses of 1000-2000 IU are generally safe.
mild
Steroids (corticosteroids) — Long-term steroid use depletes vitamin D. If you take prednisone or similar medications you likely need higher doses.
Ask your doctor to test your vitamin D level if you are on long-term steroids.
moderate
Weight-loss medications (Orlistat) — Orlistat reduces absorption of fat-soluble vitamins including vitamin D3 significantly.
Take vitamin D3 at least 2 hours before or after Orlistat.
Rose bottom line
"This is one supplement where the evidence actually backs up the hype, especially for your bones. Get your blood level tested first - most of us are running low without knowing it. Your future self will thank you for taking this seriously now."
